A number of operational and human performance issues were identified as having caused the plane crash on Misima Island in 2010.
This was indicated in the final crash report released yesterday by the PNG Accident Investigation Commission.
Aircraft performance, aerodrome facilities, flight crew performance and experience of previous incidents were some of the factors.
